Regence BlueCross BlueShield Clinical Improvement Grant Fuels Deployment of Provider-Based Disease Management System from Kryptiq

Family Physician Group Selects CareManager to Fight Diabetes Epidemic

PORTLAND, Ore. – February 8, 2007 – Kryptiq today announced that Family Physicians Group (FPG) will be implementing a diabetic management program using CareManager™, the award-winning disease management system that empowers physicians to deliver proactive and coordinated healthcare based on existing clinical data. FPG, an affiliate of Southwest Washington Health System, will be using CareManager to aid in identifying patients who are overdue for needed tests or appointments, or not responding appropriately to treatment and requiring special attention.

The diabetic management project was fueled in part by a Clinical Improvement Grant, awarded through a competitive call for proposals sponsored by Regence BCBSO. The grant was designed to give network providers the tools they need to better serve patients and foster a sustainable healthcare infrastructure. FPG’s use of CareManager will be the catalyst for other Southwest Washington Health System affiliates to develop a structured and analytical approach to chronic care management.

CareManager uses patient data found in EMR systems, rather than claims data that is typically based on historical rather than active healthcare. By using CareManager, physicians can proactively manage patient populations to create appropriate, measurable and cost-effective intervention programs.
