Disease management tool facilitates communication, leverages physicians’ technology investments, improves quality of patient care
PORTLAND, OR – April 3, 2009 – Kryptiq Corp. has released CareManager 3.5, an enhanced version of its chronic disease management and patient-outreach solution. During this challenging economic climate, many patients are delaying or avoiding medical treatment due to high cost. More than ever, physicians must be able to reach and communicate with patients, including those missing necessary treatments, to avoid unnecessary medical complications and resulting increases in healthcare costs.
CareManager is a solution for physicians using the GE Centricity® electronic medical record (EMR) system. Along with Kryptiq’s Connect IQ® product suite, CareManager incorporates the use of collaborative technology such as secure messaging. With this, physicians cost-effectively streamline their approach to chronic care, maintain timely communication with patients and share critical preventative and intervention information.
Since 2006, physicians at Decatur Memorial Hospital in Decatur, Ill. have used CareManager to assess and manage chronic care populations, ensure patients’ compliance with treatment plans and become more active in overall care. Decatur Memorial Hospital’s use of CareManager helped its physicians to receive recognition from the NCQA for Diabetes Management in 2008.

This adds to the growing suite of modules, which includes diabetes, heart failure and stroke prevention modules, among others. Together, PQRI reporting and the chronic disease modules enable physicians to achieve improved revenue for improved care.

Physicians’ ability to submit data directly to CMS for the reporting year allows organizations to use a single submission for all eligible patients and providers. The new asthma module assures accurate identification and documentation of an Asthma Action plan, decreasing the risk of asthma-related complications by identifying treatment and monitoring opportunities and medication safety concerns. It also supports quality improvement efforts by providing an automated system for monitoring asthma-related performance measures.
